Claude Code 2.1.294 fixes instruction-shaped blocking hooks

The patch fixes prompt and agent hooks written as instructions allowing actions they should block. It also improves judgment of instruction-shaped Stop and SubagentStop hooks. If you depend on a hook saying “Block commands that…”, upgrade and test an actual blocked command, plus the stopping condition. ³
