Claude Code adds an explicit fail-closed hook option

Command and HTTP hooks can set onFailure: "block" so startup failures, timeouts and unexpected exit codes block the action. Configure this deliberately for gates you require, and test it by making the checker unavailable. The release also fixes remote MCP recovery after longer outages. ²
