Plannotator fixes decisions from a stale Pi review tab

Approving or denying a review left open across /reload or a session switch could crash Pi through a stale-context exception. The patch guards delivery and preserves the recorded decision; it also tells supported review tabs when the session ends. Upgrade affected setups and test a decision after reload. ⁴
