Claude Code 2.1.291 fixes lost permission answers. Upgrade before diagnosing the user.

Claude Code 2.1.291 fixes a regression introduced in 2.1.290 that could drop answers to permission prompts in cloud sessions. It also fixes a 2.1.288 regression that could lose the last session messages when quitting. The patch was published at 03:55 UTC. 1

If a task appears blocked after an approval, check the harness version and transcript first. Repeatedly granting permission can conceal a lost answer; asking the agent to continue does not tell you whether the earlier decision arrived.

On a disposable task, allow an action and watch it start. Deny another and confirm it stays blocked. Then quit and resume after a final message. Keep the version and observed results so the next upgrade has a concrete comparison.

The release notes document the vendor’s fix. These smoke checks establish whether it works in your configured cloud session.

Use it

Move affected sessions to 2.1.291 and verify approval delivery plus quit/resume behavior.
